Stuff you need to know before applying for a Z Foundation grant

• The application form must be completed by you personally as an employee of a Z Retailer
• Grants are available up to a maximum of $3,000.
• Your personal information will be kept confidential. However, you need to consent to the Trustees of the Z Foundation Trust advising your retailer of the outcome of your application.
• When considering your application, the Trustees may contact you or your Retailer and request or require further information.
• It is likely that financial help is just one part of the help you will need right now. We strongly encourage you to contact Z’s Employee Assistance Programme (EAP) to access other types of help. To apply for a grant from the Z Foundation you must:

• be currently employed by a Z Retailer; and
• have been employed by a Z Retailer for at least 90 days; and
• be directly facing unexpected and significant financial hardship. This means you are in financial difficulty because of an event or costs that you could not have expected and were out of your control; and
• answer all of the questions in the ‘Required Information’ section of this form (over the page); and
• ask your Retailer to endorse your application before you submit it. (Once you have sought your Retailer’s endorsement, you may submit your application without waiting for their decision. They will also be required to advise the Trustees directly); and
• Consent to the Trustees advising your Retailer of the outcome of your application
